29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  a) Transfer/Cooling Conveyor Belts, Presses 1 through 4 - On or About May 23, 2018 and times prior to, employees were permitted to walk and work on top of the conveyor belts that contained holes, which were not protected by covers or guardrail systems, thus exposing employees to trip and fall hazards.
