29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater.   (a)  Carpenter Shop:  On or about 18 October 2016 and times prior thereto, electrical panel CL, CP and CL/P were blocked.  This condition exposed employees to an electrical shock and burn hazard.  (b)  Maintenance Bays, Room 198:  On or about 18 October 2016 and times prior thereto, the 120/208 volt electrical panel was blocked.  This condition exposed employees to an electrical shock and burn hazard.  (c)  Warehouse Work Shop:  On or about 18 October 2016 and times prior thereto, electrical panel R3 was blocked.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  Maintenance bay, center stall:  On or about 18 October 2016 and times prior thereto, a retractable orange extension cord exhibited damaged strain relief.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Main building, Hallway area outside Associate District Manager office;  On or about 18 October 2016 and times prior there, the emergency exit light was inoperative.  This condition exposed employees to smoke inhalation and/or fire hazards from delays in existing the building.    The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.37(a)(4) which was contained in OSHA inspection number 947376, citation number 1, item number 1/a and was affirmed as a final order on 05 December 2013, with respect to a workplace located at Campbell Tract Facility, 4700 BLM Road, Anchorage.

29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm.  Outside warehouse area:  On or about 18 October 2016 and times prior thereto, approximately 12 warehouse storage racks did not have load limits posted and were not secured to the wall or floor. This condition exposed employees to a struck by hazard.  The Bureau of Land Management was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.37(a)(4) which was contained in OSHA inspection number 947391, citation number 1, item number 1 and was affirmed as a final order on 05 December 2013, with respect to a workplace located at Campbell Tract Facility, 4700 BLM Road, Anchorage.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.  Carpenter shop area:  On or about 18 October 2016 and times prior thereto, the shop area was not clean and orderly.  The work area had numerous items loosely scattered on the floor impeding the walking and working surface. This condition exposed employees to a tripping hazard.
