29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. 117 Van Cleef Drive, Readington, NJ - Regency at Readington, Building 30    The employer did not ensure employees were protected from falls by use of a personal fall arrest system.  An employee engaged in residential construction activities, working from the roof, was removing windows to install ice shielding from the second story dormers without the use of fall protection.     Violation observed on or about October 25, 2016    Arcadia Home Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 965165, citation number 1, , item number 2/A and was affirmed as a final order on June 16, 2014, with respect to a workplace located at 814 South 19th Street, Philadelphia, PA 19146.
