29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.: (a) On or about November 8, 2016, employees were exposed to unguarded ingoing nip points and rotating parts created by the DCM USIMECA Babycat re-winder located in the Plant 5 shrink department. (b) On or about October 27, 2016, employees were exposed to unguarded ingoing nip points and rotating parts created by the DCM USIMECA Sleevecut machine located in the Plant 5 shrink department.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: On or about October 27, 2016, employees were exposed to the point of operation created by the cutter bar of a DCM USIMECA Sleevecut machine located in the Plant 5 shrink department.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. Metallic outlet boxes (handy boxes) were not installed in accordance with the UL identifier OCIT which states the boxes shall be mounted in or on a surface. The metallic outlet boxes were attached to drop down flexible cords and suspended in the air to be used to power sleeve cut machines located in Plant 5 shrink department.
