29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle. On or about November 17, 2016, an employee was exposed to an amputation hazard. The employee was cutting 1/2 inch copper tube on a Powermatic band saw. The blade guard was adjusted to approximately 3 inches. The employee did not adjust the blade guard to the height of the stock.
