29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a) Standridge Color Corporation, Social Circle, GA - In the paper coating building, the LOTO specific procedures for cleaning the paper coating lines indicated to turn off the electrical power in the main control cabinet in the motor room, turn off the 3 breakers for the specific coating line labeled unwind, center and rewind. The breakers were not labeled with those names. The employees were de-energizing the line by turning main red handle disconnect on the control panel by the machine. The LOTO specific procedures did not clearly outline the steps for shutting down the line, on or about 11/17/16. b) Standridge Color Corporation, Social Circle, GA - In the paper coating building, the LOTO specific procedures for cleaning of the slitting lines indicated to turn off the disconnect valve. The disconnect was not a valve. The employees were de-energizing the line by turning the main red handle disconnect on the control panel by the machine. The LOTO specific procedures did not clearly outline the steps for shutting down the line, on or about 11/17/16.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Standridge Color Corporation, Social Circle, GA - In the paper coating building, employees would press the e-stop to stop the paper coating line and go inside the cage enclosing the extruder to clean plastic drips on the nip roller. The line was not de-energized and LOTO affixed to each energy isolating device before entering the caged area, on or about 11/10/16.
