(b) Carbon Black/Resin Extrusion System; on or about June 3, 2021 and at times prior to, employees were exposed to hazards created by the point of operation, when the employer failed to guard the ram in the back of the melt (lower) extruder.

29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Carbon Black/Resin Extrusion System; on or about June 3, 2021 and at times prior to, employees were exposed to amputation hazards, in that the employer did not ensure that a lockout/tagout procedure was utilized to protect workers engaged in clearing jams in the melt (lower) extruder. Standridge Color Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1470226, citation number 1, item number 3a and was affirmed as a final order on June 18, 2020, with respect to a workplace located at 455 Hood Road, Jasper, GA 30143.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Carbon Black/Resin Extrusion System; or about June 3, 2021 and at times prior to, employees expected to lockout extrusion equipment were exposed to electrical energy, hydraulic power, and compressed air where the employer failed to include all required steps and information in the lockout/tagout procedures such as but not limited to identification of the equipment, scope and purpose, and list of all hazardous energy sources the employee could be exposed to. Standridge Color Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1470226, citation number 1, item number 3b and was affirmed as a final order on (date), with respect to a workplace located at 455 Hood Road, Jasper, GA 30143.
