29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  a. On or about December 19, 2016, at the library basement/bottom stairway area, a designated exit route door was locked from the inside due to a door knob that was defective and non-operational.     b. On or about December 20, 2016, at the chemical laboratory/basement area, a designated exit route door was locked from the inside due to non-operational push bar.   NOTE: Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a. On or about December 15, 2016, at the Forestry technicians cubicle/GIS bldg., an uninterruptible APC 700 power supply was not directly connected to a permanently installed branch circuit receptacle.  b. On or about December 19, 2016, at the Technical Information Specialist cubicle/Library bldg., an uninterruptible APC Pro 1000 power supply was not directly connected to a permanently installed branch circuit receptacle.  c. On or about December 19, 2016, at the Directors' work area/Library bldg., an uninterruptible APC Pro 1000 power supply was not directly connected to a permanently installed branch circuit receptacle.  d. On or about December 19, 2016, at the Purchasing Agent's cubicle/Procurement Department, an uninterruptible APC 750 power supply was not directly connected to a permanently installed branch circuit receptacle.  NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

On or about December 19, 2016, at the library building, all self luminous exit signs located above the exit doors and exit routes were not operational and were not replaced on or before the manufacturers' date of September 2004.    NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).
