29 CFR 1926.451(a)(6): Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section.    a.  At the worksite located at 1455 W. 3rd Ave., Columbus, Ohio, on the second level, below the most northern faux truss, employees designed, constructed, and worked from a wooden makeshift scaffold that was not compliant with the scaffolding requirements of 29 CFR 1926 Subpart L in that adequate guardrails were not installed, thereby exposing employees to an approximately 14 foot fall hazard.      b.  At the worksite located at 1455 W. 3rd Ave., Columbus, Ohio, on the second level, below the second most northern faux truss, employees designed, constructed, and worked from a wooden makeshift scaffold that was not compliant with the scaffolding requirements of 29 CFR 1926 Subpart L in that guardrails were not installed and unsecured plywood was used for a platform, thereby exposing employees to an approximately 14 foot fall hazard.

At the worksite located at 1455 W. 3rd Ave., Columbus, Ohio, on the second level, below the second most northern faux truss, employees worked from a wooden makeshift scaffold that had unsecured plywood placed on top of unsecured lumber, thereby exposing employees to an approximately 14 foot fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.     a.  At the worksite located at 1455 W. 3rd Ave., Columbus, Ohio, on the second level, employees installed the most northern faux truss from a makeshift wooden scaffold without the use of fall protection, thereby exposing employees to an approximately 14 foot fall hazard.      b.  At the worksite located at 1455 W. 3rd Ave., Columbus, Ohio, on the second level, employees installed the second most northern faux truss from a makeshift wooden scaffold without the use of fall protection, thereby exposing employees to an approximately 14 foot fall hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.    a.  At the worksite located at 1455 W. 3rd Ave., Columbus, Ohio, at the top of the Southern stairway there was a floor hole approximately six inches in diameter, thereby exposing employees to a tripping hazard.
