29 CFR 1926.350(a)(9): Compressed gas cylinders shall be secured in an upright position at all times except, if necessary, for short periods of time while cylinders are actually being hoisted or carried. a. On and/or before March 9, 2020, in room 105 of the 7 story building, three acetylene cylinders were not secured to prevent them from being knocked over and damaged while in storage.

29 CFR 1926.350(a)(11): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage places shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. a. On and/or before March 9, 2020, in room 105 of the 7 story building, three acetylene cylinders were stored adjacent to combustible cardboard boxes containing supplies/materials and not located where cylinders would not be knocked over or damaged by passing or falling objects.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. On or about March 9, 2020, in the apartment 407 bedroom of the 4 story building, the duplex receptacle which was being used to energize the Utilitech portable light (model: 6290XL; Mid-Ohio #998) did not have a faceplate in addition to other uncovered duplex receptacles in the room.
