29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     On December 16, 2016, on the center north face of the construction project, employees were working from tube and coupler scaffolding that was not fully planked. Employees were exposed to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.     On December 16, 2016, on the center north face of the construction project, access ladders or other means of access were not provided for employees working from tube and coupler scaffolding at levels from 10 feet to 80 feet.  Employees were exposed to fall hazards.

29 CFR 1926.452(b)(2): Transverse bracing forming an "X" across the width of the scaffold was not installed at the scaffold ends and at least at every third set of posts horizontally (measured from only one end) and every fourth runner vertically. Bracing did not extend diagonally from the inner or outer posts or runners upward to the next outer or inner posts or runners:    On December 16, 2016, on the center north face of the construction project, workers were working from tube and coupler scaffolding where the transverse bracing across the width of the scaffold was not installed. Employees were exposed to fall hazards.
