29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):     a.) On March 22, 2017, inside the canopy at the main casino entrance, an employee fell from a scaffold platform which was not fully planked.   b.) On March 24, 2017, inside the canopy at the main casino entrance, employees working from scaffolding that was not fully planked were being exposed to fall hazards.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.    On March 24, 2017, inside the canopy at the main casino entrance, employees working from scaffold planks extended 20-inches over the cantilevered portion of the platform exposing employees to fall hazards.

Bracing did not extend diagonally from the inner or outer posts or runners upward to the next outer or inner posts or runners:    On March 24, 2017, inside the canopy at the main casino entrance, employees were working from tube and coupler scaffolding without transverse bracing installed across the width of the scaffold. Employees were exposed to fall hazards.
