29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 130 Boykin Lakes Blvd., Pike Road, Alabama:  On or about January 12, 2017 and at times prior; the employer allowed his employees to work from the top of the ceiling joist on the second floor without fall protection exposing them to a fall hazard of 20 feet 4 inches to the concrete floor below.  b)  At 130 Boykin Lakes Blvd., Pike Road, Alabama:  On or about January 12, 2017 and at times prior; the employer allowed his employees to work from the top of the ceiling joist on the first floor without fall protection exposing them to a fall hazard of 11 feet to the concrete floor below. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  a)  At 130 Boykin Lakes Blvd., Pike Road, Alabama:  On or about January 12, 2017 and at times prior; the employer allowed his employees to use a ladder to gain access to and from the second floor and the rails of the ladder did not extend at least three feet above the landing to provide the employees with safe access exposing them to a fall hazard of 11 feet 4 inches to the concrete floor below.
