Westfield Electroplating Company, 68 N Elm St. Westfield, MA 01085: On or about 01/17/2017, employees wore 3M Series 6000, half facepiece respirators on a voluntary use basis while cleaning up a chemical spill from a Sulfuric and Nitric acid waste tank. The employer did not ensure that employees were medically able to wear a respirator, and that the respirators were cleaned, stored, and maintained so that the respirators did not present a health hazard to the users.
