29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.       a) front of house - employees were exposed to a fall of 11 feet 4 inches while using a straight ladder to access the roof. The ladder only extended about one foot above the landing, on or about 2/2/17.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a)  roof - employees were exposed to a fall of 22 feet while installing a roof on a 5 in 12 pitched roof, in that, fall protection was not used, on or about 2/2/17.    b) lower roof - an employee was exposed to a fall of 11 feet 4 inches while installing a roof on a 3 in 12 pitched roof, in that, fall protection was not used, on or about 2/2/17.      Brad McDonald Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1004881, citation number 1, item number 1 and was affirmed as a final order on 12/11/2014, with respect to a workplace located at 7053 Peregrina Loop, Wesley Chapel, FL, 33545.    Brad McDonald Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 997669, citation number 1, item number 1 and was affirmed as a final order on 11/25/2014, with respect to a workplace located at 206 S Moody Ave, Tampa FL 33609.    Brad McDonald Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1004881, citation number 2, item number 1 and was affirmed as a final order on 5/21/2015, with respect to a workplace located at 4347 Vermillion Sky Drive, Wesley Chapel, FL 34554.
