29 CFR 1910.303(b)(2): Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.  a) Human Resources Building, File Room:  On or about February 7, 2017 and at times prior thereto, two Belkin surge suppressers were connected, in series, exposing employees to electrical or fire hazards.  b) Casino, IT Maintenance Shop, at two desks:  On or about February 7, 2017 and at times prior thereto, two surge suppressers were connected into another surge suppresser, exposing employees to electrical or fire hazards.  c) Casino, Slot Technician Office:  On or about February 7, 2017 and at times prior thereto, underneath the office desk, two surge suppressers were connected, in series, exposing employees to electrical or fire hazards.  d) Casino, Slot Technician Office:  On or about February 7, 2017 and at times prior thereto, two surge suppressers were connected, in series, energizing several battery charging stations, exposing employees to electrical or fire hazards.   Note:  Abatement certification is required for this item.

(a) Casino, Server room: On or about February 7th 2017, and at times prior thereto, the first electrical panel on the left when entering into the server room did not have any labeling indicating which breaker energizes specific equipment in the casino.  Electrical/shock hazard if employee turned wrong breaker in the event of an emergency.

29 CFR 1910.37(b)(4): A directional sign was not posted along the exit access indicating the direction of travel to the nearest exit  and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent  (a) Casino, hallway behind Tule Grill kitchen: On or about February 7th, 2017 and at times prior thereto, the northern-most end of the hallway leading out of the back of the Tule grill, did not have a directional exit sign to indicate the path of travel to the exit door.  This exposed employees to smoke inhalation hazards.
