29 CFR 1910.146(g)(1): The employer shall provide training so that all employees whose work is regulated by this section acquire the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under this section.    The employer had not ensured that employees entering the furnaces (FBT) to perform work had not received training on permit-required confined spaces  in a language necessary to acquire the understanding, knowledge and skills for safe performance.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    Employees who performed servicing and maintenance on the Furnaces were not required to:  (1) affix lockout devices to the electrical energy source.  (2) prior to starting work on machines that have been locked out, verify that isolation and deenergization of the furnace had been accomplished.    a.  During the daily changeover and replacement of rollers in the Glass Bending Area, furnace operators are exposed to a struck-by, caught in-between hazard from the drop of the rollers.    b.  On February 11, 2017, while changing heating elements in FBT 1, Furnace, employees who entered the furnace to install the heating elements did not lockout the furnace and verify that the electrical power was shut off to the furnace before starting work.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    In the EPBL 2 Print Room, a Print Operator was exposed to a struck-by, caught in-between hazard from the squeege and bar on the print table when it moves across the screen during operation and during inking and cleaning of the screen due to the lack of a safeguard to protect the operator.
