This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.  On or about at 2/27/17 located 5315 Whisper Point Blvd., Cumming, GA. Employer failed to provide abatement of  all three violations (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about at 2/27/17 located at 5315 Whisper Point Blvd., Cumming, GA: Employees were stocking the roof with bundles of shingles on to the roof with a 8/12 pitch of a house that was under construction while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately 45- feet.    J & H Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926. 501(b)(13), which was contained in OSHA inspection number 989594, citation number 1, item number 1, and was affirmed as a final order on 10/27/14, with respect to a workplace located at 1511 Silver Mist Cir. Lot # 176 Powder Springs, GA
