LP gas cylinder. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Third Floor - On or about February 27, 2017, a worker was exposed to a 10 foot fall from a wooden job-made ladder whose top was located 1 foot below the roof or ladder landing. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Roof - On or about February 27, 2017, a worker installing capping on the parapet of a flat roof was not provided with fall protection while exposed to a 35 ft. fall. b) Roof - On or about February 27, 2017, a worker installing capping on the parapet of a flat roof was exposed to a 10 ft. fall through a 2 foot by 4 foot skylight opening which was not provide with a cover. c) Roof - On or about February 27, 2017, a worker installing capping on the parapet of a flat roof was exposed to a 10 ft. fall through a 2 foot by 2 foot opening or floor hole which was not covered. d) Third Floor - On or about February 27, 2017, a worker was exposed to a 7 foot fall while walking near to an open-sided floor next to a stairway where no fall protection was provided.
