29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:    a)  Roof - On or about February 27, 2017, a worker who had not been provided with a portable fire extinguisher was exposed to a fire hazard while operating a torch fueled by a 20 lbs. LP gas cylinder.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a)  Third Floor - On or about February 27, 2017, a worker was exposed to a 10 foot fall from a wooden job-made ladder whose top was located 1 foot below the roof or ladder landing.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   a)  Roof - On or about February 27, 2017, a worker installing capping on the parapet of a flat roof was not provided with fall protection while exposed to a 35 ft. fall.     b)  Roof - On or about February 27, 2017, a worker installing capping on the parapet of a flat roof was exposed to a 10 ft. fall through a 2 foot by 4 foot skylight opening which was not provide with a cover.     c)   Roof - On or about February 27, 2017, a worker installing capping on the parapet of a flat roof was exposed to a 10 ft. fall through a 2 foot by 2 foot  opening or floor hole which was not covered.    d)  Third Floor - On or about February 27, 2017, a worker was exposed to a 7 foot fall while walking near to an open-sided floor next to a stairway where no fall protection was provided.     Oscar Cruz Construction dba Master Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1123662, citation number 1, item number 1 and was affirmed as a final order on 05/19/2016, with respect to a workplace located at 1048 48th. Pl., NE, Washington, DC 20019.   Oscar Cruz Construction was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1001984, citation number 1, item number 2 and was affirmed as a final order on 02/04/2015, with respect to a workplace located at 2841 Belair Rd., NE, Washington, DC 20007.   Oscar Cruz Construction was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1042573, citation number 1, item number 1 and was affirmed as a final order on 04/17/2015, with respect to a workplace located at 911 12th. St., NE, Washington, DC 20002.   Oscar Cruz Construction was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1071280, citation number 2, item number 1 and was affirmed as a final order on 12/15/2015, with respect to a workplace located at 816 Kennedy St., NW, Washington, DC 20011.   Oscar Cruz Construction was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1073461, citation number 1, item number 1 and was affirmed as a final order on 10/13/2015, with respect to a workplace located at 422 58th. St., NE,  Washington, DC 20019.   Oscar Cruz Construction was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1018431, citation number 2, item number 2 and was affirmed as a final order on 04/16/2015, with respect to a workplace located at 411 Ridge St., Washington, DC 20001.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a)  Stairway, First to Second Floor - On or about February 27, 2017, a worker was exposed to up to a 10 foot fall down a set of stairs having six risers which had not been provided with a hand rail.   b)  Stairway, Second to Third Floor - On or about February 27, 2017, a worker was exposed to up to a 10 foot fall down a set of stairs having six risers which had not been provided with a hand rail.
