29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. Framing employees working at elevations greater than 6 feet above the ground or lower levels while performing commercial construction of wood frame buildings are not being protected from fall hazards by a fall protection system. The employer fails to assure effective fall protection against falls as required by the standard as most recently demonstrated on March 3, 2017, on the 6:12 pitch roof of the North Bay Haven Elementary School in Panama City, when three framing employees were exposed to fall hazards of 13 to 30 feet from the eaves and through the open trusses, in that they were not protected by a fall protection system. Action Concrete Construction, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard, 29 CRF 1926.501(b)(1), which was contained in OSHA inspection number 1074107, Repeat Serious citation number 02, and was affirmed as a final order on December 29, 2015, with respect to a workplace located at 7718 Front Beach Road, Panama City Beach, Florida. Action Concrete Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 911300, Serious citation number 01, item number 001, and was affirmed as a final order on September 19, 2013, with respect to a workplace located at 522 Mary Esther Cutoff, Mary Esther, Florida. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including and explanation of how these steps protect its employees from fall hazards.

On or about March 3, 2017, framing employees that were exposed to 13 to 30 feet high fall hazards at the North Bay Haven Elementary School had not been trained to properly set up personal fall protection systems.
