F/V Prowler, OSI Dock; on or about 07 March 2017 and times prior thereto;  two instances were noted where Bradley eyewash stations were not maintained in accordance to manufacturers requirements; quick drenching or flushing of the eyes was not provided within the work area for immediate emergency use.             a. Processing area; Eyewash station was not sanitary. Eyewash station had fish waste in the wash basin.        b. Engine room; Eyewash station was not sanitary. Eyewash station had salt and a dry-erase marker in the wash basin.  This condition exposed employees to eye trauma hazards.
