29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve. OSI Dock, F/V Bering Prowler, Engine Room: On or about 10 February 2018 and times prior thereto, 3M Half mask respirator was not properly stored for protection. This exposed employees to respiratory hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: OSI Doc, F/V Bering Prowler, Factory: On or about 10 February 2018 and times prior thereto, The eye wash station was blocked with factory equipment. This condition exposed employees to eye trauma hazards.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels. OSI Dock, F/V Bering Prowler, Hauling Station: On or about 10 February 2018 and times prior thereto, employees were exposed to over 89.6 decibels on a daily basis and were not receiving annual hearing tests. This exposed employees to occupational noise exposure hazard without follow-up evaluations to test for hearing loss.
