29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). F/V Bering Prowler, OSI dock, upper aft weather deck, on or about 10 February 2018 and times prior thereto; the employer did not ensure that unguarded edges were effectively protected. An unguarded edge measuring approximately 9-feet above the lower surface, did not have guardrails. This condition exposed employees to a fall hazard.

29 CFR 1915.113(b)(3): Hooks shall be inspected periodically to see that they have not been bent by overloading. Bent or sprung hooks shall not be used. F/V Bering Prowler, OSI dock, weather deck, on or about 10 February 2018 and time prior thereto; the employer did not ensure that rigging gear was inspected periodically. A four-bridle nylon messing lifting assembly; comprising of four nylon mesh strands with attached hooks; had one hook noticeably sprung. This condition exposed employees to a struck by hazard.
