29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): a) Machine #9 - 12; On or about March 8, 2017 the employer exposed employees to amputation hazards in that employees were required to troubleshoot - test or position - the overwrap carriage on Machine #9 - 12 while the machine was energized. The employer failed to follow the required sequence of actions as set by the standard when equipment is energized for testing or positioning. Employees were not removed from the machine or equipment area prior to the machine being energized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Maintenance Shop; On or about March 14, 2017 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to operate the Sharp Lathe without a chuck guard. b) Production Floor; On or about March 14, 2017 and at times prior thereto the employer exposed employees to amputation, caught-in, and struck-by hazards in that employees were allowed to operate including but not limited to the Shirring Machine #9 - 3 with unguarded rotating parts near the point of operation. c) Production Floor; On or about March 14, 2017 and at times prior thereto the employer exposed employees to amputation, caught-in, and struck-by hazards in that employees were allowed to operate including but not limited to the Shirring Machine #9 - 4 with unguarded rotating parts near the point of operation.
