29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: Plant: An employee who performed electrical work on machines, equipment and premises wiring did not wear appropriate personal protective equipment such as fire resistive clothing, voltage rated gloves, a face shield and arc-rated balaclava when performing diagnostic testing, including when using test equipment to verify that equipment was properly de-energized. In addition, the hair net provided for use in the plant was not rated to be used around electrical equipment.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Weld Shop: On, or about, March 28, 2017, the tongue guard provided on the left side of the no-name bench mounted off-hand grinder was adjusted one half of an inch from the abrasive wheel.
