29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Extruder #1 Laminating Stand:  On, or about, March 16, 2023, the extruder was not locked out in accordance with the company's lockout/tagout procedures when performing the task of cleaning the surface of the chill roller. The employer did not enforce the energy control procedure as operators routinely cleaned the high-speed chill roller during normal production operations. This cleaning practice exposed employees to moving machine parts and in-going nip point hazards created by the in-running chill roll pinch point.
