29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Extruder #1 Laminating Stand: On, or about, March 16, 2023, the extruder was not locked out in accordance with the company's lockout/tagout procedures when performing the task of cleaning the surface of the chill roller. The employer did not enforce the energy control procedure as operators routinely cleaned the high-speed chill roller during normal production operations. This cleaning practice exposed employees to moving machine parts and in-going nip point hazards created by the in-running chill roll pinch point.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition or applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: Extruder #1 Laminating Stand: On, or about, March 16, 2023, employees did not receive specific training on Extruder #1 Laminating stand when performing servicing and maintenance activities on equipment including, but not limited to, cleaning the surface of the chill roller, which had multiple energy sources including electric, compressed air, and water exposing employees to the in-running chill roll pinch point.
