29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Extruder #1 Laminating Stand: On, or about, March 6, 2020, an employee suffered a serious hand injury when he was pulled into the in-running chill roll nip point while attempting to remove waste material from the extruder die. Adequate safeguarding was not provided in the area of the chill roll to prevent access to the hazardous area of the machine.
