(A) Cook Engine Warehouse - On or about March 29, 2017 and at times prior thereto, the CSHO observed a Delta Unisaw(serial 99L55163) circular crosscut table saw that was not guarded by a hood as required.

29 CFR 1915.73(d): When employees are exposed to unguarded edges of decks, platforms, flats, and similar flat surfaces, more than 5 feet above a solid surface, the edges shall be guarded by adequate guardrails meeting the requirements of 1915.71(j)(1) and (2). (a) Cook Engine - boat storage area - parkking lot - M/V Expeditions - On or about March 29th, 2017 and at times prior thereto, the CSHO observed a 8' Werner A-Frame ladder installed for access to M/V Expeditions. While on the swim platform, employees were exposed to a fall of 6' to the pavement. A barbed wire fence was located within 2' of the back of swim platform. The fall to the lower level, behind the barbed wire fence, was approximately 10'.

29 CFR 1915.72(a)(3): Portable ladders shall be lashed, blocked or otherwise secured to prevent their being displaced. The side rails of ladders used for access to any level shall extend not less than 36 inches above that level. When this is not practical, grab rails which will provide a secure grip for an employee moving to or from the point of access shall be installed. (a) Cook Engine Inc. - boat storage area - parking lot - On or about March 29th, 2017 and at times prior thereto, the CSHO observed a Werner 8' A frame ladder in place to access the swim platform of M/V Expeditions without being secured to prevent being displaced.

29 CFR 1915.134(e): When safety guards are required, they shall be so mounted as to maintain proper alignment with the wheel, and the guard and its fastenings shall be of sufficient strength to retain fragments of the wheel in case of accidental breakage. The maximum angular exposure of the grinding wheel periphery and sides shall not exceed 180 degrees. (a) Cook Engine- warehouse- On or about March 29th, 2017 and at times prior thereto, the CSHO observed a DeWalt 4 1/2" angle grinder model D28402 (serial # 438471) without a guard installed around the abrasive wheel. (b) Cook Engine- warehouse- On or about March 29th, 2017 and at times prior thereto, the CSHO observed a DeWalt 4 1/2" Heavy Duty Cordless Cut-Off Tool model DC411 without a guard installed around the abrasive wheel.
