The rated load of the crane was not plainly marked on each side of the crane, and if the crane has more than one hoisting unit, each hoist shall have its rated load marked on it or its load block and this marking shall be clearly legible from the ground or floor. a) Workshop - On or about December 7, 2022 and at times prior thereto, employees were exposed to a struck-by and/or a crush hazard by using a crane to lift loads without labeling that identified the safe lifting capacity (load rating).

29 CFR 1910.179(k)(2):Rated load test. Test loads shall not be more than 125 percent of the rated load unless otherwise recommended by the manufacturer. The test reports shall be placed on file where readily available to appointed personnel. a) Shop - On or about December 7, 2022 and at times prior thereto, the employer did not have any documentation or proof of a rated load test of the crane. This exposed employees to struck-by and/or crushed by hazards in that the crane may be damaged internally, which may not be identified by a daily visible pre-use inspection. Without an official load test, the crane may fail resulting in a load drop.

Oxygen and fuel gas pressure regulators including their related gauges were not in proper working order while in use. a) Shop - On or about December 7, 2022 and at times prior thereto, the broken/missing cover for oxygen regulator exposed employees to inadequate pressure readings. This hazardous condition is unlikely to result in death or serious bodily harm.
