Delaware St., Denver, CO 80223: On or before April 6, 2017 the employer did not conduct annual inspections of the energy control procedures at least annually to ensure that the procedures and requirements of this standard were being followed for equipment such as, but not limited to, the slitter machines. The slitter machines were operated with multiple energy sources, including electrical and pneumatic energies. Condition exposed employees servicing the equipment to laceration hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts: (a) Protecto Wrap Company, at 2255 S. Cherokee Street, Denver, CO 80223: On or before April 6, 2017 the employer did not ensure that machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip pints and rotating parts in the following instances: 1) The operator was not protected from ingoing nip points of the Line One Slitter Machine when loading and connecting the roller, and when activating the machine. In addition, the foot control was not guarded to prevent inadvertent startup; 2) The second employee receiving the completed rolls on the Line One Slitter Machine was not protected from ingoing nip points; and 3) The operator, the second employee, and other employees were not protected from the rotating shafts on both sides of the Line One Slitter Machine. These conditions exposed employees to laceration hazards.
