29 CFR 1910.212(a)(2): Guards on machines were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a. Protecto Wrap Company at 1955 S. Cherokee St., Denver, CO 80223: On and before November 10, 2021, the employer did not protect employees from hazards created by pinch points. It was observed that the rollers on the backside of the Pinnacle Slitter machine where the material is cut was not adequately guarded. This condition exposed employees to crushing hand injuries. b. Protecto Wrap Company at 1955 S. Cherokee St., Denver, CO 80223: On and before November 10, 2021, the employer did not protect employees from injury while operating Pinnacle Slitter machine. It was observed that the cutting blades behind the rollers to cut product were not guarded. This condition exposed employees to lacerations and amputations.

29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone during the operating cycle: a. Protecto Wrap Company at 1955 S. Cherokee St., Denver, CO 80223: On and before November 10, 2021, the employer directed employees to operate the Pinnacle Slitting Machine without the use of machine guards. The employer failed to protect employees from injury further in that the safety limit switch present was not operating. An employee sustained a crushing injury to their hands when reaching into the point of operation. This condition exposed employees to crushing hazards. Protecto Wrap Company., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1223122, citation number 1, item number 2 and was affirmed as a final order on September 29, 2017, with respect to a workplace located at 2255 S. Delaware St., Denver, CO 80223.
