29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors entered were not effectively closed: Trinidad Drilling Rig #138 Drill Site: On or about April 5, 2017, located on the air compressor; the opening where the electrical cable entered the pressure relief valve power unit was not effectively closed. The condition exposed the employee(s) to electrical shock hazards.

29 CFR 1910.1200(a)(1): The employer did not effectively ensure that the information concerning the hazards of all chemicals to which the employees may be exposed to, was transmitted as accomplished by the means of the employer's comprehensive hazard communication programs including labeling and other forms of warning, material safety data sheets and employee training: Trinidad Drilling Rig #138 Drill Site: On April 4, 2017; Tote container between fuel tank and parts house was not labeled. This condition exposed employee to health hazards.
