Employees performing roofing work from a 6:12 pitch roof are not being protected from 17 feet 4 inches fall hazards by a fall protection system.    The employer routinely fails to assure effective protection against falls as required by the standard, as demonstrated on April 13, 2017, when employees were re-roofing a two-story residential building at 14750 Beach Blvd., Jacksonville, Florida 32277 without fall protection and were exposed to a fall hazard of up to 17 feet 4 inches above the surrounding ground.    Stonebridge Construction Services LLC., was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(10), which was contained in OSHA inspection number 1132142, citation number 1, item number 2 and was affirmed as a final order on March 30, 2016, with respect to a workplace at 1755 Eagle Harbor Parkway, Orange Park, Florida.        Stonebridge Construction Services LLC., was previously cited for a violation of this Occupational Safety an Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 767243, citation number 1, item number 1 and was affirmed as a final order on February 1, 2013, with respect to a workplace at 138 Spartina Ave., St. Augustine, Florida.
