At a temporally worksite located at 4521 Fulton Avenue, in Jacksonville, FL: On or about March 20, 2025, the employer exposed employees to head injuries, in that protective helmets (hard hats) were not utilized by employees while passing material to the employees at the roof level.

29 CFR  1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a.  On a residential roof, at a temporary job site located at 4521 Fulton Avenue in Jacksonville, FL: On or about March 20, 2025, the employer exposed employees to fall hazards, in that the ladder rails did not extend 3-foot above the upper landing surface of the roof at the eave.

29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. On a residential roof, at a temporary jobsite located at 4521 Fulton Avenue, in Jacksonville, FL: On or about March 20, 2025, the employer exposed employees to fall hazards, in that fall protection was not utilized by employees while performing roofing work.  Stonebridge Construction Services, LLC, was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1635230, Citation Number 1, Item Number 1 and was affirmed as a final order on February 7, 2023, with respect to a workplace located at 1735 & 1737 Seminole Road, Atlantic Beach, Florida 32233.

Stonebridge Construction Services, LLC., located at 11323 Phillips Parkway Drive E, Unit 7, in Jacksonville, FL 32256: On or about March 20, 2024, the employer failed for calendar year 2024, to electronically submit information from the OSHA Form 300A or equivalent by March 2, 2025. The establishment employed approximately 41 employees and was classified in the North American Industrial Classification System as 238160 during Calendar Year 2024.
