29 CFR 1926.501(b)(10): On a low-slope roof 50 feet or less in width, Needham Roofing failed to use a safety monitor with no other responsibilities which could take the monitor's attention from the monitoring function:     a) On or about April 13, 2017, employees were exposed to fall hazards in excess of 17 feet while engaged in roofing activities for a two story townhouse complex with no fall protection in place;    b) On or about April 13, 2017, Needham Roofing, Inc., as controlling contractor, did not ensure that employees of Jose Serrano, a subcontractor, were protected from fall hazards in excess 17 feet while re-roofing a townhouse complex with no fall protection in place.      Needham Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 613398, citation number 2, item number 1 and was affirmed as a final order on February 7, 2014, with respect to a workplace located at 6105 S. Parker Road, Centennial CO, 80111.
