29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:   Building 1243- Warehouse/Salvage: On April 20, 2017 the exit was blocked with miscellaneous items which obstructed the path to the outside.  The Presidio Trust was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 957077, notice 2, item 1 and was affirmed as a final order on 5/12/14, with respect to a workplace located at The Presidio Trust, San Francisco, CA.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   Carpentry Shop Building 1750: On April 20, 2017, the electric drill, Powermatic # 10 Model #200, Serial # 4-5114  was not guarded.  It was used for drilling metal and wood.   The Presidio Trust was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 957077, notice 2, item 3 and was affirmed as a final order on 4/24/15, with respect to a workplace located at The Presidio Trust, San Francisco, CA.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  In the following locations, surge protectors were used as permanent wiring:  Building 1750- Breakroom: April 24, 2017, a surge protector with portable extension cord were used as fixed wiring for the Black and Decker Toaster Oven and the Panasonic Microwave.  Building 1353- Toads and Trails: On April 24, 2017, a surge protector was used attached to the wall near the door entrance which was used to power tools and miscellaneous equipment.  The Presidio Trust was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 957077, notice 3, item 1 and was affirmed as a final order on 4/24/15, with respect to a workplace located at The Presidio Trust, San Francisco, CA.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    Building 1241- Warehouse/Appliance: On April 20, 2017, the electric panel load center controlling the lights and outlets was blocked by appliances, the area was used for storage preventing direct access for operators and maintenance employees.   Building 1243- Warehouse/Salvage: On April 20, 2017, the electric shut off controlling the compressor and panels for lights, and Fire Control Panel was blocked by equipment including gardening tools  whereby preventing direct access for operators and maintenance employees.  Building 1355- Mechanic Shop: On April 24, 2017, the electric shut off controlling the compressor and panel load center controlling the lights was blocked by combustible material and other spill absorbent material preventing direct access for operators and maintenance employees.   The Presidio Trust was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1)(I)(A), which was contained in OSHA inspection number 957077, Notice 1, item 1, and was affirmed as a final order on 4/24/15, with respect to a workplace located at the Presidio Trust, San Francisco, CA.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   Building 1353 - Roads and Trails/Tree Crew: On April 24,2017, two electric boxes were missing faceplates. The outlets were used by the employees for maintenance tasks and they were exposed to electrical hazards.  The Presidio Trust was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(2)(i), which was contained in OSHA inspection number 957077, notice 2, item 5 and was affirmed as a final order on 4/24/15, with respect to a workplace located at The Presidio Trust, San Francisco, CA.

29 CFR 1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.
