29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Machinery throughout the production Floor:  On or about May 16, 2017 and at times prior thereto the employer exposed his employees to potentially hazardous energies in that employees were allowed to perform maintenance and servicing on injection molding machines which have multiple energy sources such as electrical, hydraulic and pneumatic without those employees having a lock out program; without being trained in lock out tag out and without written machine specific lock out procedures.

(a) Maintenance area: On or about May 16, 2017 and at times prior thereto the employer exposed employees to fire hazards in that employees were allowed to walk and work in the same area where oxygen cylinders and fuel gas (acetylene) cylinders were stored together.
