29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about May 15, 2018 and times prior machine/equipment specific procedures: The employer failed to ensure machine/equipment specific procedures were developed/available for an authorize employee who entered the robot caged area of A-4 without using proper lockout procedures to get a part that was dropped.  b) On or about May 25, 2018 and times prior machine/equipment specific procedures: The employer failed to ensure machine/equipment specific procedures were developed/available for an authorize employee who entered the mold area of B-1 without using proper lockout procedures to clean material off a mold.  c) On or about May 24, 2018 and times prior machine/equipment specific procedures: The employer failed to ensure machine/equipment specific procedures were developed/available for an authorize employee who entered the mold area of A-8 without using proper lockout procedures to check a water leak on the front of the mold.  d) On or about March 24, 2018 and times prior machine/equipment specific procedures: The employer failed to ensure machine/equipment specific procedures were developed/available for an authorize employee who entered the mold area of A-5 without using proper lockout procedures to get part(s) out of the mold due to the robot arm not working properly.  e) On or about May 24, 2018 and times prior machine/equipment specific procedures: The employer failed to ensure machine/equipment specific procedures were developed/available for an authorize employee who entered the mold area of A-5 without using proper lockout procedures to get a runner off the mold.  f) On or about May 25, 2018 and times prior machine/equipment specific procedures: The employer failed to ensure machine/equipment specific procedures were developed/available for an affected employee who entered the mold area of A-1 without using proper lockout procedures for cylinder issues on the mold.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:  a) On or about May 23, 2018 and times prior lockout/tagout devices were not assigned: The employer failed to ensure authorized employees who conduct lockout/tagout procedures were provided adequate lockout/tagout devices.  The company only has one lockout/tagout station with six locks and the current specific procedure provided by the company indicates seven locks are required to lockout mold machines.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about June 2, 2018 on B-3 mold machine: The employer failed to ensure employees were protected from the hazards associated with contact with a robot arm as two employees reached under the front portion of the robot cage to retrieve parts and push them to the operator since the conveyor belt was not running and the mold machine was still producing parts.
