Where there was only one exit access leading to an exit or exit discharge, the width of the exit and exit discharge was not at least equal to the width of the exit access.  a. Camaco, LLC located in Lorain, Ohio:  On or about May 22, 2017, the employer did not ensure the fire exit access route located in the small warehouse was unobstructed.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees.   a. Camaco, LLC located in Lorain, Ohio:  On or about May 22, 2017, the employer did not ensure the operator that was operating the Motoman NX100/EA1400N welder for cell 134, located in area P552, was trained as an authorized employee prior to performing cleaning operations on the index table clamps.  b. Camaco, LLC located in Lorain, Ohio:  On or about August 1, 2017, the employer did not ensure weld technicians  were trained to lockout the Motoman NX100/EA1400N welder for cell 134, located in area P552 before changing weld tips and cleaning weld nozzles.

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and was not done in the following sequence:  a. Camaco, LLC located in Lorain, Ohio:  On or about May 22, 2017, the employer did not ensure the Motoman NX100/EA1400N welder for cell 134, located in area P552, was locked out prior to the operator removing weld splatter from the clamps, exposing the operator to pinch points created by the pneumatically controlled clamps and the rotation of the index table.  b. Camaco, LLC located in Lorain, Ohio:  On or about August 1, 2017, the employer did not ensure the Motoman NX100/EA1400N welder for cell 134, located in area P552, was locked out prior to the weld technician entering the robot cell to change weld tips and clean the weld nozzles, exposing the employee to the rotation of the index table and the movement of the two weld robots.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     a. Camaco, LLC located in Lorain, Ohio:  On or about May 22, 2017, the employer did not ensure the Motoman NX100/EA1400N welder for cell 134, located in area P552, was properly guarded.  The light curtains guarding the front of the welder did not interrupt the movement of the welder when they were activated.

The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. Camaco, LLC located in Lorain, Ohio:  On or about May 8, 2017, the employer did not ensure the Motoman NX100/EA1400N welder for cell 132, located in area P552, was properly guarded.  An employee received an injury when the pneumatic clamp, used to hold the part, activated and pinched his fingers while the employee was loading the part into the machine.
