29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:    a). On or about June 2, 2017, at the workplace, employees were using ROMEX brand rigid 12/2 wire as temporary extension cord wire to energize portable electric hand tools being used by employees to screw metal roof panels to the roof trusses, exposing employees to electrical shock hazards.

29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:  a. On or about June 2, 2017, at the workplace, employees were using ROMEX brand rigid 12/2 wire as temporary extension cord wire to energize portable electric hand tools used by employees, and did not protect this wire from cuts or abrasions by moving equipment on the construction site, exposing employees to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a). On or about June 2, 2017, at the workplace, employees were using ROMEX brand rigid wire as a flexible extension cords to energize portable electric hand tools.  The cord was affixed to an attachment plug but had been stretched and pulled from the plug end, exposing the neutral conductor wire, exposing the employees to electrical shock hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a. On or about June 2, 2017 three employees of Pro Steel, Inc were installing screws on a metal, 4:12 pitch, low slope roof on a new 18 foot high, commercial structure 200 feet long by 130 feet wide, and were not protected from falling to the ground below by any means of guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.
