29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about June 19, 2017 an employee was exposed to caught-in hazards while attempting to straighten a sheet of paper going into the tubing machine by accessing the bottom side of the stack rollers by passing through an unsecured gate and the employer did not ensure that the appropriate guards were in place.
