29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements:  On or about June 13, 2023, employees who used the dock door stairway next to Dock 1 were exposed to fall hazards because the employer did not ensure each open side of the stairway was equipped with a handrail.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about June 13, 2023, employees were exposed to amputation, caught-in, and entanglement hazards because the employer did not ensure:  a. Tuber #3 roller adjustment gates were guarded to prevent ingoing nips and rotating parts.  Two of the three gates could not be latched, and the middle gate latched but was easily defeated without special knowledge or tools.   b. Tuber #3 was guarded near point of operation to prevent ingoing nips, entanglement, and amputation hazards.  Components such as belts, rollers, and perforators were used to cut, fold, and seal bags.

29 CFR  1910.303(g)(1)(i): The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1:   On or about June 13, 2023, employees were exposed to electrical and fire-related hazards because the employer did not ensure at least three feet of space was maintained around electrical panels and disconnects:  a. Maintenance Area South wall - one 240V electrical panel and 6 disconnects up to 480V were blocked by a wire bin and a metal drum that were being stored in front of the equipment.  b. Maintenance Area East wall - one 480V electrical panel blocked by Cybertech control unit.  c. C Building, back wall near Tuber #4 - one 208V electrical panel blocked by stored adhesive gaylord.  d. North wall next to Faustel printer - one electrical panel blocked by stored equipment.
