29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about February 27, 2024, an employee was exposed to hazardous energy while troubleshooting an air leak on the Pinch Bottomer and the employer's machine specific procedures did not include hazardous energy control procedures for the trouble shooting operation.

29 CFR 1910.147(c)(7)(iii)(B): Additional retraining was not provided whenever a periodic inspection under 29 CFR 1910.147(c)(6) revealed that there were deviations from or inadequacies in the employee's knowledge or use of the energy control procedures: On February 27, 2024, an employee was exposed to amputation hazards while troubleshooting an air leak on Pinch Bottomer Feeder without following lockout procedures and the employer did not ensure that retraining was provided due to deviations of the employee's use of energy control procedures.
