OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to: A.) Corry Rubber Corporation; Mix Area - On or about 1/3/17, and at times prior thereto, employees were potentially exposed to fire and deflagration hazards from a dust collector located inside. Dust collectors included, but were not limited to, the Celtron dust collector which was located inside the building and was not equipped with any devices to mitigate a potential dust explosion. The dust collector collected a combination of dust that included, but was not limited to, carbon black, natural rubber, sulfur, and zinc oxide. B.) Corry Rubber Corporation; Mix Area - On or about 1/3/17, and at times prior thereto, employees were potentially exposed to fire and deflagration hazards from a dust collector that was not equipped with devices and systems to prevent the propagation of deflagrations and associated flame fronts from the dust collector through the associated dust collection system duct work. Celtron dust collector Model #DSLG-10-8 was not equipped with approved devices to prevent the propagation of a deflagration and associated flame fronts throughout the dust collection system. The dust collector collected a combination of dust that included, but was not limited to, carbon black, natural rubber, sulfur, and zinc oxide

Main Street; Mixing room - On or about 1/3/17, and at times prior thereto, pallets were being stored underneath 480 volt electrical panels. The electrical panels serviced items including, but not limited to, the mixer and the dust collector.
