29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) On or about July 27, 2017 and at times prior, Westchase Construction Ltd., as the controlling employer, did not ensure that the workers of Quest Masonry, Inc., as the correcting employer, inspected the scaffold for visible defects by a competent person before each work shift. Outrigger planks were not nailed, bolted, or secured to the outriggers; each abutted end of the outer planks was not rested on a supported surface; and guardrail systems were not installed along all open sides of the platform. A foreman of Quest Masonry, Inc. and Funez Stucco LLC were working on each end of the outer planks when one of the workers moved towards the other worker. The planks tilted to one side which caused both workers to fall approximately 40 feet to the ground.
