29 CFR 1910.178(l)(6): At this establishment, the employer had not certified that each powered industrial truck operator has been trained and evaluated as required by paragraph (l) of 1910.178, with a certification that includes the name of the operator, the date of the training, the date of the evaluation, and the identity of the person performing the training and evaluation.  On or about August 8, 2017, the employer failed to ensure that certification and recordkeeping of the training of powered industrial vehicles was took place. Employees had received training and evaluation of operation however this was not certified.
