Adequate machine guarding was not in place to prevent exposure to point of operation hazards.

29 CFR 1904.39(a)(2): The employer did not report within twenty-four (24) hours, the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident. At the establishment, a work-related injury occurred and the employer failed to report to OSHA the inpatient hospitalization prior to the elapse of the 24 hour reporting period of the incident and thereafter.
