29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) Employees manufacturing orthotic inserts at 246 Columbus Ave in Roselle NJ use grinding, polishing and buffing wheel machines to shape, smooth and clean the orthotics.  Personal monitoring measured exposure to noise at 280%, 140% and 137% Dose; equivalent to 8-hour Time-weighted average of 97.4 dBA and 92.4 dBA and 92.3 dBA respectively.  The employer did not implement a hearing conservation program for employees with noise exposure exceeding the values in Table D-2, to include training & audiometric testing, both initially and at least annually thereafter.    Condition(s) were noted on or about 9/06/2017.
