29 CFR 1910.147(c)(6)(i)(D): Where tagout was used for energy control, the periodic inspection did not include a review between the inspector and each authorized and affected employee of the employee's responsibilities under the energy control procedure:  At the time of the inspection, the employer had not conducted a periodic inspection with each employee authorized and affected employees who use locks and tags for energy control. The employer last conducted a review during November 2012.

29 CFR 1910.212(a)(2): Guard on machine was not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  At the time of the inspection, the slide gate guard on the film applicator robot on line #12 was not affixed. Employees were able to easily lower the guard exposing themselves and others to a caught in hazard during robot operation. At times the robot had been operated with the guard down.
