On or about August 5, 2017 On a 6:12 pitch roof an employee performing roofing work did not wear protective foot wear and was exposed to puncture and laceration injuries to his feet.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a. On or about August 4, 2017 employees were conducting tear off operations of roof shingles, skylights, and underlay materials an existing 6:12 steep slope roof and were not protected from falling more than six feet to the surface below by stepping into skylight openings by any means of personal fall arrest systems, covers, or guardrail systems erected around such holes, the employee were exposed to an 8-foot fall hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a. On or about August 3 2017 and daily through August 8, 2017 employees were stripping old roof materials from a 6:12 pitch steep slope residential roof and were not protected from falling more than 6 feet to the ground below by any means of personal fall protection, guardrails or safety nets, the employees were exposed to an 8-foot fall hazard.
